Also, please read the “Privacy Practices” and “Disclosure Statement” and feel free to print or save them for your reference. Included in the first appointment paperwork are signature pages stating that you received both of these documents and consent to treatment. Make sure to bring these signature pages with you and we will talk further about this information at the appointment. Thank you!
